Incident and Injury investigations

Most organisations believe that the investigation of injuries is important to prevent recurrence and as a learning process to monitor the overall health of the organisations to prevention strategies.

In most cases there is a significant gap between this belief and what happens in reality. In practice organisations are generally good at reporting injuries because of workers compensation and insurance concerns but incident investigation is often cursory or too narrowly focussed on apportioning blame.

This often results in many of the causes of the incident being ignored because they are overlooked or do not fit into the scenario required by the investigator. Factual statements obtained by insurance companies often suffer from the problems above and are also often carried out a significant period of time after the accident when the site and circumstances are a distant memory.

  1. Training in incident investigation
    This course can be tailored for your organisation, and the time available, and provides a simple but comprehensive approach to investigating all incidents and injuries. Participants will be able to identify the multiple causes of most incidents using a simple diagrammatic tool. Examples covered in the course can include actual examples from client organisations as well as looking at well-known incidents including Piper Alpha.

  2. Professional investigation of major incidents or accidents

    When you have a major incident or injury you need a trained professional to assist in obtaining a rigorous analysis of the events leading up to the incident or injury. RiskChase consultants will work with you and your legal adviser to obtain the truth behind the events and also assist in providing expert advice that may be required. As an independent group we can demonstrate to other stakeholders that the investigation is strictly focussed on obtaining access to the facts. Our reports are carefully considered and targeted at prevention of recurrence.

  3. Routine investigation of injuries or incidents

    Organisations are rapidly recognising the benefits of obtaining their own external investigation when faced with routine injuries. These benefits include demonstrating a commitment to prevention, consultation with the workforce and retention of information for possible future use.
